Planning a Patient's Flight

Steps to follow in planning a flight:

• Call our Mission Coordinator between 8:00am and 5:00pm Central Time Monday thru Friday.  She can answer any questions you might have and make a preliminary determination as to whether or not we can accommodate the patient's needs.
   

• If the mission is within our capabilities, she will fax to you several forms and a check list that will explain the information that we will need about your patient. All completed forms and required patient information must be in our hands no later than 4 PM Central Time two days prior to the scheduled mission date.
